Click the patient information tab, select self schedule an appointment, then select your appointment. Wait (at least 48hrs) to receive a patient portal invite via email  to Register your account. Once you log in, your account will become active. You may sign your enrollment consents, and complete required documents.Your appointment will be confirmed after all required documents are completed.
